How to register

We have an open list and welcome requests for registration from patients living in or moving to the practice area.  We also are happy to take on patients who live out of the area or if you move out of our catchment area, but will ask you to complete an Out of Area form and you won’t be able to have a visit from the GP or nurse if one was needed.  There may be circumstances where we are not able to have you remain registered when out of area due to a change in care needs and will discuss this with you.

You will need to complete these forms by hand and bring it to the surgery.  We aim to register you with the surgery within 10 days of you bringing the form in.  Please make sure you have a month’s supply of medication from your previous surgery so you aren’t without during this time.

You will have a named, accountable doctor who is responsible for coordinating your care. You can still talk to or make appointments to see any of our doctors or nurses, not just your named GP.

Temporary patients

If you are not registered with a GP you will be unable to see one unless you have a serious emergency. In most other cases, calling 111 is a better option.
